My name is Sarah Harris, the Certified School Nurse for the Wellsboro Elementary Schools. I oversee day to day nursing care in the Don Gill Elementary School, while Nicole Miller, LPN, oversees day to day nursing care in the Charlotte Lappla Elementary School.

I have been working as a Certified School Nurse for the Wellsboro Area School District since 2015. I graduated from Mansfield University with a Bachelor’s degree in Nursing in 2012 and obtained my school nursing certification from Slippery Rock University in 2016. I completed my Masters of Education in School Health Services in 2022. In addition to school nursing, my career has included various hospital and community health settings.

Guided by the PA Department of Health and PA Department of Education, my goal as a school nurse is to provide a safe, healthy environment for your children as they learn. The nursing services at Wellsboro Area School district strive to meet the individual health needs of each child so they can perform optimally in the school setting.

Medication at School

The following medications are considered “standing order medications” and only require one parent permission slip that must be filled out at the beginning of each school year and upon enrollment for new students.

All other medications require that students have the following form filled out and signed by their physician and parent/guardian for each prescribed medication that is to be administered by the school nurse during the school day. Students will not be allowed to receive medication at school without this form.

Mandated Dental Health Program (Kindergarten and 3rd Grade)

Students in Kindergarten and Third Grade are required to have a dental examination by a registered dental hygienist. Students who have had a dental examination during the school year or within four months prior to the start of the school year (after April 1) may submit documentation of a private dental exam that has been signed by their dentist. The following form must be filled out and returned to the school nurse for students in Kindergarten and Third Grade to be exempt from a dental screening at school.

PA State Law requires that all students be up to date on immunizations prior to the first day of school.

Immunization requirements for school entry

PA State Law allows for students to be exempt from immunizations for medical, religious, or moral/ethical reasons. Students who are exempt from immunizations must have the following form on file with the school nurse by the start of the school year.

Physical Examination (Kindergarten)

The Pennsylvania Public School Code Section 1402. Health Services (e) requires that students upon original entry, sixth grade and 11th grade be given a "comprehensive appraisal" of their health. The physical is to be completed within 4 months prior to the start of the school year (after April 1). In addition, Section 1402(c) requires the completion of medical questionnaires which are to become part of the student's health record.

This requirement may be completed by the student's primary care provider (MD, DO, CRNP, PA) or by the school provider.

Students must have a private physical form on file with the school nurse in order to be exempt from the school physical examination.
