Section 504

Students are eligible for accommodations through a Section 504 Service Agreement if they have a disability that interferes with his/her ability to learn or to access school programs.

Initial 504 Process: Dr. Jake Rogers (K-6) & Dr. Ben Largey (7-12)

  • Issue Permission to Evaluate to Parent & Parent Input Form

  • Request any records necessary to support the need for accommodations/obtain appropriate release of records as needed

Upon receipt of signed Permission to Evaluate:

  • Issue Teacher Input Form

  • Complete the Student Eligibility Form to determine eligibility

  • If a student is eligible prepare Section 504 Service Agreement & Meeting Invitation

  • Schedule a meeting to discuss the Section 504 Service Agreement

    • Parent, School Psychologist/LEA, General Education Teacher

  • Obtain parent signature for services on Section 504 Service Agreement

  • Communicate with all teachers of the student that the Section 504 Service Agreement is in place

Annual Section 504 Service Agreement Review Process:  Dr. Jake Rogers (K-12)

  • Contact parents to see if anything has changed in regards to student (medical needs, updates to current level of support, etc.)

  • If the student continues to be eligible:

    • Update the current Section 504 Service Agreement and obtain parent signature (formal meeting or other)

    • Communicate with all teachers of the student that the Section 504 Service Agreement is in place

  • If the student is no longer in need of accommodations:

    • Request input from teachers/parents to indicate

    • Communicate with parents via phone call or meeting

    • Obtain parent signature to discontinue services
