These are the forms that are required for each medication your child will be taking at school. Please keep in mind, over the counter medications STILL REQUIRE these forms.

The following medications are considered “standing order medications” and only require one parent permission slip that must be filled out at the beginning of each school year and upon enrollment for new students. If you need a new permission slip for these medications, please click here.
